| Puppy Sales Agreement |
| Seller agrees that this puppy or dog is in good health to the best of my knowledge at this time. Puppy has been checked by my vet and a health certificate has been given to the buyer as well as a shot and worming record and feeding instructions.
Buyer MUST take this puppy or dog to the vet within 72 hours after leaving my premises (unless further arrnagements were made such as a holiday and agreed upon by both parties, stated in this contract, and initialed by both parties) or this contract will be void. If any life threatening health defects are found, Seller much be notified within 24 hours with a full statement from the vet as to the health defect. Puppy will only be replaced with one of equal value (when one becomes available), only if it has been determined to have a genetic defect that will cause death to the puppy. Not covered are allergies, thyroid dysfunction, cherry eye, hernia (cherry eye and hernia are not hereditary problem but caused by an injury), or any other autoimmune disorders that may be caused by environmental facotrs. The guarantee does not cover any skin or eye problems that may develope later after the sale of the puppy or dog. Also not covered are parasites, internal or external, (puppy has been on a regular worming schedule and information given to buyer), or parvo virus. NO REFUNDS WILL BE GIVEN AT ANY TIME. This agreement is only good until the puppy reaches the age of one year. Buyer agrees to take good and reasonable care of this puppy or dog and for maintaining a routine shot and worming schedule at their expense. Buyer also agrees that if he does not desire to keep this puppy or dog for any reason they will contact Seller to give them the first priority to claim or repurchase said dog or puppy. Buyer forfeits all rights to any moneys until Seller can resell the dog, at which time any advertising, board, or vet bills will be deducted from the total. If Seller finds out that the said puppy or dog is being abused or neglected, Seller has the right to reposes this puppy or dog and the Buyer will sign over the registration papers to Seller immediately. | If you purchase one of my puppies and you live out of state, two copies of this contract will be mailed to you, one will be signed by me and that is for your records, you will sign the other one and send it back to me for my records. |
